Sliding Scale fee available with proof of income

Insurance
As a Licensed Professional Counselor, I am an in-network provider for Blue Cross/Blue Shield, United Behavioral Health, Cigna, Aetna, TriCare and more. I am an out-of-network provider for most other insurance companies. If you wish to obtain reimbursement from your insurance company according to their out-of-network benefit schedule, I will be happy to provide you with a completed insurance form to submit to them. It is recommended that the individual wanting to submit a claim to their insurance provider first call about their benefits and verify out-of-network coverage for mental health services by a Licensed Professional Counselor.

Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

• Do I have mental health insurance benefits?
• What is my deductible and has it been met?
• How many sessions per year does my health insurance cover?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician?

Receipts not submitted for insurance claims may be tax deductible under medical expenses. Check with your tax preparer.

Payment
Cash, check and major credit cards accepted for payment.

Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session.
